Also required

Not registry lookups; ezOIG doesn't run these
  • BCI criminal records check including FBI information at hire, and every four years (OAC 5122-30-31(C)(8), (C)(9))
  • Employment application, reference checks, signed no-conviction attestation and agreement to report new charges (OAC 5122-30-31(C)(2), (C)(7))
  • Valid driver's licence and BMV driving record for staff who transport residents (OAC 5122-30-31(C)(6))

Why some registries are not screened yet

The registry blocks automated access
The registry's website turns away automated lookups. We don't work around that, so for now your staff search it themselves.
The registry's terms forbid automated searches
The registry's terms, or the law behind it, rule out automated or third-party searches, so ezOIG doesn't run them.
Requires an employer account
Searches go through the employer's own account with the agency, so ezOIG doesn't run them for you.

Guidance, not legal advice

This page summarizes ezOIG's research into Ohio's rules for this program, checked against the regulations as of October 16, 2026. Rules change, and an agency's own contract or license can add to them. Confirm what applies to you with Ohio Department of Medicaid or your own counsel before relying on it.
