Also required
Not registry lookups; ezOIG doesn't run these- Sworn statement or affirmation disclosing criminal convictions or pending charges (12VAC30-120-930 B 19 c; Code of Va. § 32.1-162.9:1)
- Two references from prior jobs with no evidence of abuse, neglect or exploitation (12VAC30-120-930 B 19 a)
- Personal care aide training of at least 40 hours, or nurse aide certification or an approved curriculum (CCC Plus Waiver Manual, Chapter II)
- Drug-free workplace policy for licensed home care organizations (Code of Va. § 32.1-162.9:1 C)
Why some registries are not screened yet
- The registry blocks automated access
- The registry's website turns away automated lookups. We don't work around that, so for now your staff search it themselves.
- Checked by request to the agency
- The agency answers a written request or form rather than an online search.
- Paid access
- The agency charges for access, and ezOIG doesn't include it yet.
Guidance, not legal advice
This page summarizes ezOIG's research into Virginia's rules for this program, checked against the regulations as of October 16, 2026. Rules change, and an agency's own contract or license can add to them. Confirm what applies to you with Department of Medical Assistance Services (DMAS) or your own counsel before relying on it.
