Also required

Not registry lookups; ezOIG doesn't run these
  • Fingerprint-based state and FBI criminal history check for direct care positions, sponsored residential providers and their household adults, shared living partners and direct care contractors (Code of Va. § 37.2-416 B)
  • Applicant disclosure of convictions, pending charges and founded child abuse or neglect (12VAC35-105-400 B)
  • Criminal history records check for consumer-directed employees through the DMAS fiscal employer agent within 30 days (12VAC30-122-120 A 15 a)
  • Private drivers: valid licence, insurance, driving record and a signed criminal history attestation (12VAC30-122-440)
  • Tuberculosis screening for consumer-directed assistants (DMAS DD Waivers Manual, Chapter II)

Why some registries are not screened yet

The registry blocks automated access
The registry's website turns away automated lookups. We don't work around that, so for now your staff search it themselves.
Checked by request to the agency
The agency answers a written request or form rather than an online search.
Paid access
The agency charges for access, and ezOIG doesn't include it yet.

Guidance, not legal advice

This page summarizes ezOIG's research into Virginia's rules for this program, checked against the regulations as of October 16, 2026. Rules change, and an agency's own contract or license can add to them. Confirm what applies to you with Department of Medical Assistance Services (DMAS) or your own counsel before relying on it.
