MI Choice Waiver

Paid staff and volunteers of waiver agencies and direct providers who enter participants' homes (community living supports, chore, respite, nursing), and self-determination workers. Adult day health is a MI Choice service, but the criminal history rule covers staff entering homes.

Also required

Not registry lookups; ezOIG doesn't run these
  • Reference checks before authorizing an employee or volunteer to serve in a participant's home (Medicaid Provider Manual, MI Choice Waiver 9.4)
  • Self-determination workers: 18 or older, trained in universal precautions and blood-borne pathogens (Medicaid Provider Manual, MI Choice Waiver 4.6.A.4)

Why some registries are not screened yet

The registry blocks automated access
The registry's website turns away automated lookups. We don't work around that, so for now your staff search it themselves.

Guidance, not legal advice

This page summarizes ezOIG's research into Michigan's rules for this program, checked against the regulations as of October 15, 2026. Rules change, and an agency's own contract or license can add to them. Confirm what applies to you with Michigan Department of Health and Human Services (MDHHS) or your own counsel before relying on it.
