Assisted Living (CBRF and RCAC)
Employees and contractors with regular, direct resident contact at community-based residential facilities (ch. DHS 83) and residential care apartment complexes (ch. DHS 89). Merged because both follow the caregiver law; the RCAC rule also names the DOJ and registry checks at hire.

Also required
Not registry lookups; ezOIG doesn't run these- Background Information Disclosure form F-82064 completed at hire and every 4 years, or a written self-disclosure policy with annual notice (Wis. Stat. § 50.065(6)(am))
- Good-faith effort to obtain other states' criminal records for anyone who lived outside Wisconsin in the 3 years before the search (Wis. Stat. § 50.065(2)(bm))
- Clerk of courts records (criminal complaint and disposition) when a serious-crime charge has no final disposition or certain recent convictions appear (Wis. Stat. § 50.065(2)(bb))
- Caregivers must report new convictions or investigations to their supervisor by the next working day (Wis. Admin. Code § DHS 12.07(1))
- CBRF: TB and communicable disease screening within 90 days before employment (Wis. Admin. Code § DHS 83.17(2))
- RCAC: staff training in safety procedures, first aid and tenant rights (Wis. Admin. Code § DHS 89.23(4)(d))
Why some registries are not screened yet
- The registry blocks automated access
- The registry's website turns away automated lookups. We don't work around that, so for now your staff search it themselves.
- The registry's terms forbid automated searches
- The registry's terms, or the law behind it, rule out automated or third-party searches, so ezOIG doesn't run them.
- Not open to the public
- The agency doesn't publish it. Only the agency, or employers it authorizes, can check it.
- Paid access
- The agency charges for access, and ezOIG doesn't include it yet.
Guidance, not legal advice
This page summarizes ezOIG's research into Wisconsin's rules for this program, checked against the regulations as of October 16, 2026. Rules change, and an agency's own contract or license can add to them.
