  • Initial health evaluation and current CPR certification, if applicable, for direct care staff (77 Ill. Adm. Code 295.3000(f))
  • Hire and separation dates entered in the Health Care Worker Registry within 30 days, and employment verified there at least annually (225 ILCS 46/33(i); 77 Ill. Adm. Code 955.145)
