    Providers need to be aware that it is their responsibility to verify that the individual is not on a federal sanctions list. Thus, it is essential that providers regularly check (i.e., monthly) the federal sanctions list, which is at https://exclusions.oig.hhs.gov/.

  • Tuberculosis skin test within two weeks of hire (R432-725-7(3))
  • Personal care aides at least 18, trained in first aid and able to demonstrate competency (R432-725-14(1))
  • Medicaid EPAS service coordinators, financial management agencies and assessors must each pass a Utah criminal history and background check (Utah Medicaid Personal Care Services manual, §§ 2-1 to 2-3)
